Why We Built CHC
Communities need tools that reflect how coordinated housing access actually works. Too often, systems rely on disconnected processes, workarounds, and limited visibility across providers.
We built CHC to support coordinated entry as a connected system. It brings key parts of the process together, strengthens alignment across providers, and supports more transparent movement from first connection through referral and tracking.

We built CHC with direct input from frontline staff and with participant experience in mind.

Participants are not the primary audience for this site, but they remain central to how CHC works. CHC helps communities build systems that stay organized, understandable, and more responsive to the people moving through them.
Built from Real-World Expertise
CHC was created by Community Rebuilders, a nonprofit organization based in West Michigan with decades of experience in homelessness services, coordinated entry design, and system operations. That real-world expertise shapes how CHC works.
CHC does not force communities into a generic software model. It supports the realities communities face every day, including changing policies, limited housing inventory, evolving funding requirements, and the need for clear communication across partners.
Looking Ahead:
GRACE Network
Community Housing Connect is part of a broader vision for how communities coordinate access, resources, and care across systems.
We are developing Grace Network, an upcoming expansion of this work that will support broader coordination across systems. Grace Network will build on the same foundation as CHC—shared visibility, stronger alignment, and a more connected experience.
